Jimson Weed - Uses and Side Effects

Jimson weed is most commonly used as dried leaves, with or without tips of flowering branches. The ripe seeds and flowers without leaves are also used. Seeds are small, long, flat, and dark yellow to brown. Jimson weed's primary action isanticholinergic, caused by 0.1 % to 0.6% atropine, hyoscyamine, and scopolamine. All parts of the plant contain these compounds, but the highest concentration is in the seeds. Anticholinergic levels in other plant parts vary from year to year and from plant to plant. The alkaloids are readily absorbed across GI mucous membranes and across the respiratory tract. Anticholinergic effects usually occur within 60 minutes and may last 24 to 48 hours because of impaired GI motility.

Safety Risk Jimson weed is listed as an unsafe herb by FDA. It isn't recommended for routine therapeutic use.

Reported uses

Jimson weed is used to treat asthma and cough from bronchitis or influenza, usually by smoking cigarettes made from the leaves. It's also used to treat disorders of the autonomic nervous system. Little data exist to support routine therapeutic use of jimson weed.

Illicitly, the seeds have been chewed, the leaves smoked as cigarettes, and a tea brewed and ingested to cause hallucinations and euphoria.

Administration

Dosage and administration aren't well documented.

Hazards

Adverse reactions to jimson weed include headache, confusion, hallucinations, agitation, emotional lability, motor incoordination, restlessness, loss of consciousness, hyperthermia, tachycardia, dilated pupils, blurred vision, photophobia, dry mucous membranes, nausea, vomiting, decreased GI tract motility, dry mouth, urinary retention, tachypnea, dry, flushed skin, and hypertension leading to hypotension.

Additive effects may be seen with anticholinergics such as benzotropine, atropine, scopolamine; with antihistamines such as diphenhydramine; with phenothiazines such as prochlorperazine and promethazine; and with tricyclic antidepressants such as amitriptyline and imipramine. Use of jimson weed with deadly nightshade (belladonna) may cause additive anticholinergic toxicity.

Patients who are pregnant or breast-feeding should avoid use. Those with glaucoma, benign prostatic hyperplasia, urinary retention, tachycardia, or hypersensitivity to jimson weed should also avoid use.

Safety Risk Jimson weed has been associated with seizures, arrhythmias, respiratory depression, and respiratory arrest. Fatal poisonings resulting from respiratory depression and circulatory collapse have been reported from adult doses equal to 10 mg of atropine (15 to 100 g of dried leaves or about 100 [15 to 25 g) of seeds). Fatal doses in children may be much smaller. Fatalities also reported with ingestion of brewed tea.

Clinical considerations

Warn patient that jimson weed isn't recommended for routine therapeutic use.

Don't confuse jimson weed with deadly nightshade (Atropa belladonna), which has similar effects.

Tell patient to report signs and symptoms of anticholinergic toxicity: dilated pupils, impaired vision, dry mouth, heart palpitations, dizziness, confusion, hallucinations, and incoordination.

The antidote for anticholinergic toxicity is physostigmine. To avoid profound cholinergic effects, use it only for severe toxicity, including seizures, severe hypertension, severe hallucinations, life-threatening respiratory depression, or arrhythmias. Avoid using sedatives or phenothiazines to treat toxicity because they may have additive anticholinergic effects.

Advise patient to keep jimson weed away from children and pets.

Tell patient to notify pharmacist of any herbal and dietary supplements that he is taking when obtaining a new prescription.

Advise patient to consult his health care provider before using an herbal preparation because a conventional treatment with proven efficacy may be available.

Life Improves at 60

After I turned 60, an unexpected event occurred that inspired me to write this article. Since the age of 10, I had worn contact lenses to correct for my extreme nearsightedness.

But in the last few years, that solution had begun to work less well. After turning 56, I found myself wearing reading glasses a good bit of the time to do my work. I developed glaucoma about the same time and had to use eye drops that left my eyes sore, swollen, and dry.

Contact lens wearing became increasingly difficult. By the evening, I would just take out my lenses and read without glasses or contact lenses by pressing the book up against my nose. I was heading back to where I was at age 9 before anyone knew I couldn't see very well.

When I was 59, I remembered that my mother had promised me as an 11-year-old that someday I wouldn't have to use anything to correct my vision. I thought I might have glimpsed the fulfillment of her promise a few days later while watching Tiger Woods play golf at a local tournament.

The great golfer had had laser surgery to correct his vision a few years earlier. I was startled to see him staring steely-eyed at the flags while strong winds blinded me with dirt that interfered with my contact lenses.

At my next eye appointment, I asked my delightful ophthalmologist, Dr. Miriam Dougherty, if someone with glaucoma could have laser surgery for vision correction. She said sure and recommended I see one of the top laser surgeons in the country, Dr. Peter Rapoza.

I was delighted to hear the news and even happier when Dr. Rapoza's assistant informed me that the first visit was free to check to see if I was a good candidate for the surgery. Dr. Rapoza was a tall, distinguished-looking man who somewhat resembled the actor George Clooney. He obviously liked people and did everything he could to be kind and helpful. You feel comfortable in his presence no matter what he has to tell you.

Peter had good and bad news for me. I could certainly use the laser surgery to improve my vision, but I also had cataracts that were so bad they should be immediately removed. He recommended that I have cataract surgery and later follow up with laser surgery if anything wasn't visually perfect after the cataracts were gone.

He explained that during cataract surgery a new lens is implanted in each eye that corrects distance vision and many people find that solution avoids the need for laser corrections. I subsequently had both eyes operated on and unexpectedly ended up with the best vision of my life.

I could now see 20/20 (or with average vision) at a distance. My near vision improved so much that for most close-up tasks I could see better without reading glasses than with them. It was better than being physically reborn in a way . . . except with improvements.

With cataracts everything is dark and yellow. After the surgeries I could see that snow was still white, whereas before the surgeries I had been very concerned about the way pollution had made snow so yellow! In addition, night lights previously blurred everything into a big amorphous halo.

Now I could see unlit objects quite clearly with only starlight to help me. I woke up every day to see everything perfectly. That had never happened before in my life.

This astonishing personal experience taught me that progress can occur faster than even optimists realize. In my lifetime, technology improvements had taken me from being a virtually blind person to someone who has almost flawless vision at age 60 without any personal effort.

Let me talk about optimism for a moment. Researchers have observed that optimists often accomplish more than realists and pessimists. Why? Optimists try more things and stick to their efforts longer. The realist or pessimist often stops short of trying or trying again, just at the brink of success.

But it's not enough just to be an optimist. You need to do something based on your optimism. Optimists who never do anything except expect the best results are often referred to as day dreamers. Extreme optimists who think about what's maybe over the rainbow are often dismissed as crackpots, or worse.

I'm a different kind of optimist: an intensely practical one. I want to see results . . . and soon. I'm impatient. I do everything fast and wish I could go faster: There is so much that needs to be done that most people ignore.

That's one reason I was so impressed by my experience with cataract surgery; here was practical progress that helped me tremendously and happened on its own. Wouldn't it be great if great improvements in all kinds of areas I care about occurred without my effort?

Identifying Dog Worms and their Symptoms

There are 5 different types of dog worms (internal parasites), which your dog can fall prey to. These are: heartworm, hookworm, roundworm, tapeworm and whipworm. As a dog owner, I recommend that you educate yourself on these worms in order to be able to recognize the symptoms if they should become evident in your dog.

Early worm detection is important because each type of dog worm requires a different form of treatment. You should also be aware of the fact that roundworm and hookworm are zoonotic, which means that these worms can be transmitted to humans. Some worm infestations may show little to no symptoms, whereas others can demonstrate severe symptoms, such as vomiting, diarrhea and weight loss. Some dog worms can be seen by the naked eye while some cannot, therefore it is a good idea to ask your vet to perform a stool test for dog worms once a year.

Below you can find a list of common telltale symptoms of dog worm infestation, but keep in mind that these symptoms can also be a sign of a more serious problem. Either way, always consult a veterinarian whenever your dog is not "his usual self."

ROUNDWORMS (Toxocara canis, Toxascaris leonina)

There are two types of Roundworm: Toxocara canis and Toxascaris leonine. This is the most common type of dog worm, which affects the intestines and causes a pot-bellied appearance, mostly in puppies. There are a few ways puppies can become infected. Puppies are often infected before birth through the mother's uterus or through her milk. Puppies can also contract these worms through the ingestion of an infected animal (such as a rodent) or infected soil. Roundworm eggs can live in soil for many years. Once a puppy has ingested the infected soil, the eggs will hatch in his intestines, allowing the worms to live there and grow to adulthood. These adults will then produce more eggs.

Roundworms may be found in your dog's stool or vomit. They can grow to about 7 inches in length and have a spaghetti-like appearance. If this type of infestation is not detected early and is allowed a chance to develop, a buildup of worms in the intestines can cause an obstruction which may result in death. The symptoms of a severe infestation are: pot-belly appearance, diarrhea, vomiting, dull coat and weight loss.

Puppies should be dewormed every 2 weeks between 2 and 12 weeks of age, then monthly until he is 6 months old. Once your puppy has reached 6 months of age, he is less susceptible to contracting these worms but should continue yearly exams (or more often if considered high-risk).

Because Roundworm can be transmitted to humans, it is important to promote good hygiene, such as washing your hands regularly. Transmission of this dog worm to humans is usually through infected soil, which may be in your backyard or front lawn. Because the eggs are sticky and can easily adhere to hands or clothing, make sure children (and adults) wash their hands after playing outside (especially at a park or playground), after playing with the dog and before it's time to eat.

Prevent your dog from contracting roundworms by cleaning up fecal matter from the backyard as often as possible. Due to their sharp teeth, they also cause bleeding in the intestines. Hookworms are not visible by the naked eye, therefore should be diagnosed by a vet. As with roundworm, hookworms also live and grow to adulthood in the intestines. They can also be transmitted to pups while in the mother's uterus or through her breast milk. A dog infected with hookworm would experience bloody stool, anemia, weight loss, pale gums, diarrhea and low energy level. Skin irritation can be a sign of a severe infestation.

Hookworms can be transmitted to humans by penetration of the skin, making it is possible for people to become infected simply by walking barefoot on infected soil. Hookworms, when transmitted to humans, can cause bleeding in the intestines along with abdominal pain and diarrhea.

Treatment usually consists of oral medications (dewormer), follow-up fecal exams, intravenous therapy and, if necessary, a blood transfusion. Hookworm infestation can kill your pup before the worm is ever detected. This is why it is so important to keep up with veterinary visits and exams.

TAPEWORMS (Dipylidium caninum)

The tapeworm gets its name from its long, flat, tape-like appearance. It is yet another parasite that affects the intestines, and like the roundworm, can be seen by the naked eye. Broke pieces of this dog worm would be found in the dog's fecal matter, which give it a rice-like appearance. These pieces of worm, although broken, can be found (still moving) around the dog's anus, in his stool or in his bed. Common symptoms of severe tapeworm infestation are abdominal pain, nervousness, severe itching around the anus, vomiting and weight loss.

Transmission to dogs is often caused by the ingestion of infected fleas. Although, humans are susceptible to being infected, a dog cannot transmit the dog worm to a human directly.

Regular over-the-counter deworming medication is not effective in eliminating this type of dog worm. A prescription dewormer is administered orally or by injection (praziquantel or epsiprantel). Consult your vet. Try 1-800-PetMeds - America's Pet Health Resource

WHIPWORMS (Trichuris vulpis)

Whipworms are long, thin (whip-shaped) dog worms that live in the dog's colon and are not visible by the naked eye. They attach themselves to the intestinal walls and feed off of them which, in turn, causes intestinal bleeding. Common symptoms of whipworm infestation are anemia, weight loss, flatulence, diarrhea with blood or mucus in the stool and lack of energy.

Although whipworms are the most difficult to eliminate among the families of dog worms, there is effective treatment available.

Whipworm is most effectively treated with fenbendazole (panacur), but febantel can also be used. Prescription medications are usually more effective. The treatment lasts for up to 5 days and is repeated after 3 weeks. After this treatment is finished, consult your vet about recommending a heartworm medication (containing milbemycin oxime) as a prophylactic to future infestation. Along with administering heartworm medication regularly, here are other ways to prevent reinfection:

Remove feces from backyard every few days

Have feces tested every 6 months (more often if previously infected)

HEARTWORMS (Dirofilaria immitis)

Heartworm, although highly preventable, has the potential to be fatal, if contracted and left untreated.

Heartworm is spread by mosquitoes, mostly during the warm months when mosquitoes are most active. The mosquito becomes infected from biting dogs that carry the disease. These dog worms destroy the muscle and tissue of the heart, which can cause congestive heart failure and result in death. At this advanced stage, your dog would experience the typical signs of worms, such as pot-belly, coughing, lack of energy and dull coat.

Unfortunately, there are no symptoms of this disease until it has progressed to an advanced stage. For this reason, it is important to start your dog on a heartworm preventative such as HeartGard - Prescription Heartworm medication for Dogs & Cats at 6 months of age (after the first stage deworming process is complete. General guidelines for dog worm prevention

When walking your dog in a park, picking up his feces as a standard practice not only prevents soil contamination, but also prevents the spread of many other dog diseases.

Flea control is important because fleas are responsible for the spread of tapeworms.

Most puppies find feces quite appetizing. Keep your dog away from feces: his own as well as others. This is the most common form of worm infestation.

When cleaning your dog's area, such as his bed or crate, spray it with a strong saltwater solution and let dry. This aids in the prevention of worms.

Before traveling with your dog to obscure destinations, consult your vet of the potential risks to your dog.

Avoid exposing your dog to stray animals, birds and dead rodents, which often harbor immature tapeworms that can mature inside your dog.

Contact your vet if your dog displays any symptoms after receiving worm medication.

The optomap Retinal Exam provides a panoramic view of the back of a person's eye (the retina). The view provided by the optomap displays up to 80% of the retina without dilating the patient's eyes.

How Does the Optomap Work

The patient places their eye to be photographed up to the instrument and the doctor's assistant positions the patient for a photograph and takes the picture. The optomap captures its image in only 1/4 of a second. The photographer takes two photos and evaluates the images. If the images look good the same thing is repeated on the second eye. The image captured allows the doctor to view up to 80% of the retinal fundus at one time. The image may be magnified or manipulated for a better view. The dilated view afforded the doctor of a very photosensitive patient is often poor because it is difficult for the patient to keep their eyes open or to look in the appropriate direction. The optomap captures an image with a brief flash that even light sensitive patients do not object to, allowing the to doctor view the results without discomfort to the patient. The optomap allows for a more comfortable retinal exam. The optomap can be used as a retinal health screening tool, allowing the doctor to focus the dilated retinal exam on areas that require a better view or further investigation.

When comparing the doctor's view of your eye through an undilated pupil, with an instrument called a direct ophthalmoscope, approximately 10% of the retinal fundus is viewable. However, to attain this view requires a cooperative patient with good fixation. A wider view is achieved, through a dilated pupils, with an instrument that is worn on the doctor's head called a binocular indirect ophthalmoscope (BIO). To attain this wider view, requires a cooperative patient, with good fixation and 20 minutes to allow the drops to dilate the pupils. The patient's eyes will remain dilated for 4 to 6 hours and they may experience some blurriness during that time. By having the patient look in different directions, while the doctor readjusts his or her light and a lens that they hold in their hand, the view given by the binocular indirect ophthalmoscope can be increased to almost 100% of the retinal fundus. One of the biggest advantages of a dilated retinal fundus exam with a binocular indirect ophthalmoscope is that it allows the doctor to see the fundus in 3D or a "stereo view". A good retinal camera will allow the examiner to see about 30% of the back of a patient's eye at one time. By taking numerous images and piecing them together up to 60% of the fundus is viewable with a retinal camera.

For Whom is the Optomap Recommended

The short answer is almost everyone is a good candidate for the optomap Retinal Exam. We find, however, that some of the best candidates for the optomap are those patients that can't or don't want to be dilated, children, anyone with a history of retinal problems and people that are light sensitive.

What is the treatment for my dry eyes?

If you are experiencing any of the above symptoms you should start by buying over the counter "artificial tears" to keep your eye most. If that does not help, you will need to visit your eye doctor. More than likely they will prescribe you a topical steroid to put on your eyes. This is the first step in treating your dry eye discomfort.

If you are already experiencing infections or other vision related issues, your doctor may recommend a plug in your tear duct. These work by acting causing more tears to pool in your eyes. This is only done In very extreme circumstances.

Other possible solutions before you visit your doctor are:

· Try blinking more often. This is especially true if you use a computer for long periods of time.

· Try using a humidifier to moisturize the air. For many suffers this helps as the problem is related to dry air from the heating or air conditioning system.

· Drink more water. Make sure you aren't suffering from dehydration.

· Take your vitamins. Make sure you take your Vitamin A and fatty omega-3 vitamins. These support your eye health as you age.

The good news is that there are effective treatments for dry eye. Try some of our suggestions and you might find that a doctor visit isn't necessary.

Jaborandi is obtained from dried leaves of Pilocarpus microphyllus. It contains volatile oils and three alkaloids: pilocarpine, isopilocarpine, and pilocarpidine. Pilocarpine, a parasympathomimetic, is the primary constituent and contributes to the herb's cholinergic properties, including salivation, perspiration, miosis, and increased GI tract motility.

Reported uses

Jaborandi was formerly used to induce sweating and diarrhea, but it's no longer used as a medicinal herb. It is, however, currently used to produce pilocarpine, and approved by the FDA for treating glaucoma.

Administration

Oral use of jaborandi is unsafe. Pilocarpine, a jaborandi constituent, is commercially available by prescription as an ophthalmic solution in various strengths.

Hazards

Adverse reactions to jaborandi may include seizures, hypotension, nausea, vomiting, diarrhea, bronchospasm, dyspnea, increased sweating, and hypersalivation.

Because jaborandi has teratogenic effects and promotes uterine stimulation, pregnant and breast-feeding women shouldn't use it. Oral consumption isn't recommended.

Clinical considerations

Patient with cardiac and circulatory diseases is particularly sensitive to adverse CV reactions.

Because of potential toxicity, jaborandi isn't recommended for oral or topical use.

Symptoms of toxicity can develop after ingestion of 60 mg or more of jaborandi, which is equivalent to 5 to 10 mg of pilocarpine.

Don't confuse this herbal product with P. jaborandi (Pernambuco jaborandi) or P. pennatifolius (Paraguay jaborandi).

Always advise pregnant and breast-feeding patient to avoid using jaborandi.

Warn patient not to take jaborandi before seeking medical attention because doing so may delay diagnosis of a potentially serious medical condition.

Tell patient to notify pharmacist of any herbal and dietary supplements that he's taking when obtaining a new prescription.

Advise patient to consult his health care provider before using an herbal preparation because a conventional treatment with proven efficacy may be available.

Safety Risk

If patient shows signs and symptoms of toxicity, such as bradycardia, bronchospasm, cardiac arrest, seizures, hypotension, dyspnea, nausea, vomiting, diarrhea, increased sweating, and hypersalivation, discontinue use. If toxicity develops, prepare for gastric lavage followed byadministration of activated charcoal and atropine. Expect to give diazepam if seizures develop. Give intravenous fluids as directed if hypotension occurs. Patient also may undergo hemodialysis.

How does your garden grow? To start growing oregano, you need to know which variety you want to sow. There are culinary and ornamental types of oregano. You cannot go wrong growing oregano, it is simple to cultivate, harvest, and consume!

For growing oregano for use in the kitchen, there are two common varieties:

'Greek Oregano' is used in Mediterranean style cooking. We associate this type most with oregano flavor.

'Wild Marjoram', Origanum vulgare, is popular in French cuisine; it has a less intense odor and sweeter taste.

Different varieties of growing oregano have purple or pink clusters that bloom in June. This perennial herb grows well in most temperate climates.

You may be surprised to learn that oregano is a member of the mint family. The Greek word oregano means "joy of the mountain." Oregano grew like wildfire on the hills in Greece. The shepherds encouraged the growth for their grazing flocks, believing the white flowered herb made the meat tender and tasty.

Plant oregano seeds in a worked bed, and cover lightly with well-drained soil. You can start seeds indoors and transplant when temperatures remain above 45 degrees F. This herb does not tolerate cold weather. Oregano plants can be placed in window boxes during the winter.

Make sure the ground does not retain water.

Do not add compost or fertilizer to the soil because it will alter the taste of the growing herb.

Do not over water. When growing oregano you can let the ground dry a tad, and then water lightly. You do not want the ground to be soaked.

If you cannot furnish a well-drained bed for the plants, try growing oregano in pots instead.

Place the oregano plants a foot apart. This flavorful herb needs at least eight hours of full sun each day.

Oregano may grow two foot high but normally it averages a height of ten to twelve inches.

Growing Oregano Tips

Do not use mulch on oregano plants that will retain moisture such as wood chips, straw, or grass. Using stone mulch helps prevent root rot.

Pinch off the flower heads if your plant begins to bloom to give you more oregano leaves at harvest time.

When the oregano has reached a height of around five inches, you can begin harvesting by pinching the leaves off the plants.

Cut the growing plant back about three inches continually throughout the season to encourage new growth.

Oregano Benefits

The most beneficial medical uses of growing oregano are for respiratory infections, arthritis, glaucoma, and hypertension.

For colds or flu, a small handful of oregano in a quart of boiling water is a natural remedy. Not only does this aromatherapy vaporize a head cold but also it leaves your skin soft and moisturized.

Five Tips to Living With a Blind Dog

When the diagnosis and realization comes that your dog is blind, a type of shock ripples through the mind and heart. Then come the questions: What do I do? How do I cope? How does my dog cope? The good news is a blind dog and its owner can successfully travel the journey; this article will explain some of the ways.

Each year hundreds of dogs lose their sight, either from disease, accidents, or just old age. Progressive Retinal Atrophy (PRA), diabetes, glaucoma, automobile accident, or fireworks disaster can rob a dog of its sight. The understanding of having a special needs dog may rob an owner of his/her confidence. However, that understanding, and the ultimate decision to move forward, can also result in a greater bond between dog and owner, a deeper connection, and a stronger sense of self and purpose in both dog and owner.

My dog Sage was only 1 ½ years old when she was diagnosed with PRA, a genetic disease of the retina. I had adopted Sage from an animal shelter and I had no idea at the time that she was losing her sight. I learned many lessons along the way, how dog and owner can successfully adapt together, and how brave and determined a special needs pet can be. Living with a special needs dog is just that... special. There are adjustments both pet and owner must make, and it's up to the owner to help the dog adjust. Just as Seeing Eye dogs (guide dogs) assist blind people, so too, must a sighted owner assist the blind dog. However, blindness does not mean complete dependence. Dogs adapt quite readily to losing their sight, especially if the loss is gradual, and other senses, like hearing and smell, heighten in the animal and help the dog compensate for the vision loss. Even though a dog's adaptability and aptitude are strong, the dog owner can help in several ways.

1. Don't move the furniture - or at least don't move it often. Blind dogs learn quickly where large items are placed, and they use that memory to get around. Having the sofa in one spot for several months and then relocating it to another will cause confusion, and the dog will bump into that piece of furniture more readily when it is moved. If you do move furniture, help the dog learn the new placement by walking the dog on a lead around the room and gently brushing the dog against the piece of furniture. Help your dog understand the room's new layout by walking the dog on the lead several times. This "routine" will become set in memory.

2. Be creative with different scents and textured materials to mark specific areas in your home -- just make sure it is safe for your dog. You can use different scents of flavored extracts or even something as simple as hanging a car air freshener or potpourri sachet on a door. Using different scents in each room may help your dog distinguish different rooms in your house. Throw rugs and decorative pillows are a great help for marking different rooms of the house and for outlining where the pieces of furniture are located. For example, putting a throw rug down next to each recliner in the living room can help the dog keep from bumping into those recliners. Also, since blind dogs do run into things, and corners of tables or hallway walls can impose injury, cover these corners with padding like a soft fabric to lessen the impact on your blind dog's face or body. Additionally, indoor/outdoor carpeting, wind chimes and cedar chips or decorative bricks can help guide your blind dog along its way outside while in the yard.

3. Use bells or jingling tags on your other pet(s). Not only does this help your blind dog to find and/or follow your other pet(s), but it will also keep your blind one from being startled by the other four-footed members of your family. You can also use bells on your shoes (and those of the other members of your human family) to help your blind dog find you and to follow you more closely on walks.

4. Don't underestimate the power of gentle touch and voice. Both help reassure your blind dog. Dogs love the sound of their human's voice, and that is especially true for blind dogs. Hearing you say its name kindly and tenderly gives your dog security. Additionally, teaching new commands, such as 'step-up', 'step-down' and 'stop' (very useful for navigating stairs and taking walks!) not only keeps your dog's mind alert, but also provides extra security for you and your blind dog. Touch is calming for a blind dog, and the gentle rubs near the ears, under the chin, and on the belly remind your dog that it is loved. Affectionate voice, encouragement and training, plus tender touch provide the sense of security a disabled pet needs.

5. Sense of smell is keen in blind dogs, especially those of hunting and working breeds. Finding the food and water bowls may not be too challenging, especially if those items are kept in the same place at all times. You may want to consider using a tabletop fountain as a water bowl -- the sound of running water helps orient a blind dog and helps it find the water bowl. Some dogs like drinking from running water as well. This tip can be especially helpful if you and your blind dog move to a new home. If you decide to use this idea, get a simple fountain with a large bowl - less mess and more room for your dog's mouth and snout! So, how does one live with a blind dog? Almost exactly like one lives with a sighted dog.

There are adjustments both dog and owner must make, but as long as the dog knows it is loved and cared for, that it has a stable home with its human "pack", and that it has consistency in life, a blind dog can share a wonderful companionship with its owner for many, many years. Moving furniture in the house often is not a good idea, and catching and fetching a Frisbee probably isn't a top sport. However, living a happy life is still very possible for a blind dog. Creativity, consistency, and courage are the keys. My blind dog Sage is now nine years old and has lived with blindness for six of those years. Her tail continues to wag despite her handicap. I've grown to love and appreciate her more through the blindness experience. I respect my dog's courage, determination, and zest for life.

We share a deeply rooted bond for which I am most grateful. Traveling the pathway of blindness together, dogs and their owners can learn many things from one another. If you're like me, as a blind dog owner you will come to realize you are the one who learns the most, especially about facing obstacles and living life. It's a challenging yet incredible journey that can be successfully navigated together.

Herbal Therapy

To heal a boil, take a combination of tinctures of echinacea, cleavers, and yellow dock. Combine the tinctures in equal amounts and take 1 teaspoonful of the blend 3 times daily. Nettle tea is also beneficial. To make the tea, steep 1 to 2 teaspoons chopped fresh herb in 1 cup boiling water for 10 minutes; strain. Drink 2 cups a day.

Goldenseal may also be beneficial, as it helps cleanse the lymph glands. Take it as directed on the label, and consult your healthcare provider before using it if you have a history of diabetes, cardiovascular disease or glaucoma. Note: This herb should not be taken daily for more than 1 week, and should not be taken at all if pregnant.

You also might try adding raw garlic to your diet. Alternatively, you can take a garlic capsule 3 times daily.

An onion poultice may hasten healing. The onion should be wrapped in cloth before it is applied, and should not directly touch the boil.

Herbal products are available in health food stores and in some pharmacies and supermarkets. Follow package for specific directions.

Types of Antidepressants

Antidepressants are thought to act on neurotransmitters at the site of the synaptic cleft. Neurotransmitter release and re-uptake are the neurons' ways of communicating with each other. Low levels of neurotransmitters, in particular serotonin, noradrenaline and dopamine are thought to be associated with depressive disorders. Antidepressants are thought to inhibit re-uptake, boost production or prevent the breakdown of neurotransmitters thus boosting their action to regulate mood and alleviate depression.

There are at least 20 different antidepressants available. Although all (to some extent) will potentially alleviate the symptoms of depression, choosing the right antidepressant for a specific patient is dependent upon a number of factors such as the type of depressive disorder, possible side-effects, interactions with other medications and other medical conditions. Antidepressants may be grouped into four main classes:

1. Monoamine oxidase inhibitors (MAOIs)

2. Tricyclic antidepressants (TCAs)

3. Selective serotonin reuptake inhibitors (SSRIs)

4. Atypical antidepressants

But there are two other types of medications also used in the treatment of mood disorders: dual-action antidepressants and mood stabilisers.

Monoamine oxidase inhibitors (MAOIs) were some of the earliest medications to be used as antidepressants in the 1950s. These medications work by inhibiting the action of an enzyme called monoamine oxidase which destroys norepinephrin. By preventing the destruction of these neurotransmitters, these chemicals are able to remain in the synaptic space for a longer period of time and thus help to alleviate depression. The oldest and most common drugs of this class are Nardil (phenelzine) and Parnate (tranylcypromine) which irreversibly bind to the enzyme MAO.

However, MAO is also responsible for regulating a number of other biological processes including the metabolism of tyramine which is found in many foods, and to metabolise sympathomimetic amines which are found in many decongestants, cold and flu remedies, allergy medications and appetite suppressants. Problems, such as high blood pressure, occur when tyramine and other substances are unmetabolised causing headache, nausea, vomiting, stroke or death in some cases. Thus it is extremely important that patients disclose to their general practitioner all other medications that may be ingested.

These are some of the foods that need to be avoided in conjunction with the older MAOIs:

• All cheeses except cottage cheese and cream cheeses

• Beer and red wine, some spirits, liqueurs and nonalcoholic beer

• Soybeans, fava beans and bean curd

• Smoked, fermented, aged, or pickled fish

• Ginseng, St. John's wort, and medication containing 5-HTP

• Sauerkraut

• Shrimp paste

• Aged or processed meats such as sausage or salami

• Yeast extract or brewer's yeast

• Other foods such as some white wines and port, caffeine, chocolate, dairy products, nuts, raspberries, and spinach can only be consumed in small quantities.

Because of their potential risks, the older MAOIs tend only to be used if the other antidepressant classes have been trialled and have failed. MAOIs may be effective in the treatment of melancholic depression, when SSRIs and other narrow action and dual action drugs have failed, because of their broad range of action on the different neurotransmitter systems. Newer MAOIs, which are reversible inhibitors of the enzyme MAO, are safer and include medications such as moclobemide (Aurorix).

Tricyclic Antidepressants (TCAs) were developed in the 1950s originally as a treatment for schizophrenia. Until the advent of the SSRIs, however TCAs were the treatment of choice for depressive disorders. The TCAs which include drugs such as Tofranil (imipramine), Allegron (nortriptyline), Sinequan (doxepin) and Endep (amitryptyline) are thought to inhibit the reuptake of neurotransmitters in the synaptic cleft and thus relieve depression. They can also stimulate appetite, increase activity levels, and improve sleep. Because of their broad range of action of the different neurotransmitter pathways, tricyclic antidepressants are also particularly useful in the treatment of melancholic depression when SSRIs have failed.

While safer in some respects than the MAOIs, the TCAs are not recommended for people with glaucoma, heart disease, a history of seizures, hyperthyroidism, or urinary retention. In addition, they are contraindicated for people with liver or kidney disease. Often people complain that they feel hung over or overly sedated with the TCAs and in the elderly this side effect together with another side effect of lowered blood pressure can result in falls and fractures. Overdose with TCAs can be fatal making them unsuitable for depressed people with a high suicide risk.
If by accident they are used with MAOIs, the results can be fatal (seizures and stroke). Toxic blood levels can also occur if they are taken with antiarrhythmic drugs.

Selective Serotonin Reuptake Inhibitors (SSRIs) such as Prozac (fluoxitine), Zoloft (sertraline), Cipramil (citalopram), Luvox (fluvoxamine), Lexapro (escitalopram) and Aropax (paroxetine) act on blocking the reuptake of serotonin and thus help to alleviate depressed mood. Not only are SSRIs safer than the MAOIs and the TCAs but they also cause fewer and less disturbing side effects. However, the problems of sexual dysfunction (such as the inability to reach orgasm, erection problems, loss of lubrication in women and decreased libido) affecting up to 50% of patients following the use of this class of medications can lead to its discontinuation.
The combination of SSRIs with MAOIs is lethal but they also should not be taken with TCAs, antipsychotic, or anticonvulsant drugs, as they inhibit the liver's ability to metabolise them. Thus toxic levels of SSRIs can build up in the blood. A condition known as serotonin syndrome (described later) can develop, especially if these drugs are ingested with St John's Wort.

Dual-action antidepressants Some of the newer antidepressants target more that one neurotransmitter system (usually serotonin and noradrenaline) in the brain. Medications that fall into this group are venlafaxine (Efexor) and mirtazapine (Avanza). Venlafaxine is also thought to block the reuptake of dopamine, especially in high doses, while in low doses only the serotonin system may be affected and at medium doses both the serotonin and noradrenalin systems may be affected.

Atypical antidepressants are so named because they do not fit into the usual drug classes described above. One of these medications is Zyban (buproprion) which inhibits the reuptake of both noradrenalin and dopamine, and which is mainly used to help people quit smoking, but causes serious problems such as seizures if taken with alcohol and other adverse reactions if taken with MAOIs. People taking this medication can return abnormal ECGs and demonstrate a drop in their white blood cell count.

Mood Stabilisers Among people who are suffering from a depressive disorder within a bipolar illness, antidepressants such as tricyclics and MAOIs can cause a rapid shift into mania (referred to as Bipolar III) or increase the frequency of cycling between depression and mania. If antidepressants are used, these potential mood shifts need to be closely monitored.
There are other medications which may also be used to treat bipolar disorder including antiepileptics such as Tegretol (carbamazepine), Epilim (sodium valproate) and Lamictal (lamotrigine). Treatment with Tegretol can sometimes result in lowered concentrations of red and white blood cells and the medication can interfere with the effects of birth control pills.

Treatment Regimen
The aim of antidepressant medication therapy is to achieve the best result with the lowest dose possible and the fewest or least disabling side-effects. Starting doses for the antidepressants are usually low so that patients can be monitored for side-effects, get used to the medication regimen and gradually build up their blood levels of the drug. The earliest signs of improvement following the commencement of antidepressant treatment are mainly in sleep and appetite.

Improvements in mood occur gradually and changes are usually first noticed by friends and family. Patients usually notice an improvement in 2 to 3 weeks and the full effects of the medication should be felt by 6 weeks. Most patients will remain on antidepressants for several months even after remission of symptoms. However, the length of time that anyone remains on any one medication is dependent upon a number of factors including whether or not their stressors (if any) have resolved, if they have a strong family history of depressive disorders, and whether they suffer from recurrent bouts of depression.

The SSRIs tend to be used as first-line antidepressants and a few may be trialled before settling on one. If they are ineffective however, then the atypical antidepressants or TCAs, and more rarely the MAOIs, may be used. Sometimes, doctors may prescribe a combination of medications to achieve the best results. When switching classes of medications, patients may have to undergo a "washout" stage during which they may have to gradually reduce the dose of their original medication and experience a short drug-free period. During this time, depressive symptoms may worsen and patients may experience some withdrawal effects such as dizziness, headaches, sleep disturbances, nausea, or gastrointestinal upset.

Certain factors can cause poor drainage of aqueous fluids in the eye and this results in the build up of pressure which can cause damage to the optic nerves that helps the eye function for vision. This leads to an eye disease called glaucoma and if left untreated, it can cause permanent loss of vision. Through the years many techniques and procedures have been developed to diagnose and treat it and at this time the most promising solution is glaucoma laser surgery.

Glaucoma comes in different forms and determining what type of glaucoma a patient has helps in determining what treatment options there are. Initially, in an attempt to relieve pressure in the eye, doctors give eye drops and medication taken orally are given. If it does not help, then glaucoma laser surgery is the next option.

Argon laser trabeculoplasty is one form of glaucoma laser surgery technique used primary open angle glaucoma. Tiny burns are made into the trabecular network of the eyes to improve the draining and flow of aqueous fluid in the eyes. A specific form of this procedure is selective laser trabeculoplasty or SLT. This uses laser beams to remove only very specific target cells in the trabecular network. As a result, it improves the flow of aqueous fluids and relieves the pressure.

In some cases, it may take two sessions and in some, it may require repeating the procedure after two or three years. This, however is recommended because it avoids the need for medication after the procedure. It is also the glaucoma laser surgery procedure commonly recommended for pigmentary glaucoma which is common among males of African-American descent.

Angle closure glaucoma results if the space or angle between the iris and cornea is too narrow and this results in poor drainage of aqueous fluids. Peripheral iridotomy is the glaucoma laser surgery option for this type.

To relieve the pressure, laser beams are used to make a small hole on the iris. The space or angle widens as the iris moves away from it. This corrects the flow of aqueous fluids.

For more severe cases of glaucoma, the procedure usually recommended is laser cyclophotocoagulation. An example is when new blood vessels are formed in the eye as a result of diabetic retinopathy. The new blood vessels can seriously affect the flow of aqueous fluids in the eye resulting in pressure buildup. This leads to a form of glaucoma called neovascular glaucoma.

For this glaucoma laser surgery procedure, the doctor uses endoscopic cyclophotocoagulation to observe the ciliary body. This is also connected to an endoscopic camera. This will then guide the application of the laser beam to treat the ciliary body so it reduces or stops the production of fluids. By using a laser beam, part of the ciliary body is destroyed so that it no longer produces aqueous fluid. Less aqueous fluid means less pressure on the eye.

These glaucoma laser surgery procedures are usually painless. Some may feel a slight stinging sensation but apart from that, no major pain is felt. After undergoing glaucoma laser surgery, some rest for a few days is required and soon one can go back to the normal daily routine.

Also, in some cases, the patient needs to take some medication to relieve some minor discomforts that may appear after the procedure. Eye inflammation and slight risks in developing cataract may be some of the problems that may arise after each glaucoma laser surgery.

Glaucoma is a serious problem that can lead to permanent damage to the eye. It is important that it is well-understood and that a number of techniques are available to treat it.

Laser surgery is fast becoming a popular option for glaucoma treatment. The techniques are also becoming more and more advanced and refined to ensure that treatment results only in success and there is complete absence of discomfort during and after the treatment. They are also being improved to make the positive results long lasting, making glaucoma laser surgery a very promising form of treatment for glaucoma patients. What is glaucoma?
Glaucoma is a group of diseases that can damage the eye's optic nerve and result in vision loss and blindness. Glaucoma occurs when the normal fluid pressure inside the eyes slowly rises. However, with early treatment, you can often protect your eyes against serious vision loss.
What is the optic nerve?
The optic nerve is a bundle of more than 1 million nerve fibers. It connects the retina to the brain. (See diagram below.) The retina is the light-sensitive tissue at the back of the eye. A healthy optic nerve is necessary for good vision.
What are some other forms of glaucoma?
Open-angle glaucoma is the most common form. Some people have other types of the disease.
1.Low-tension or normal-tension glaucoma. Optic nerve damage and narrowed side vision occur in people with normal eye pressure. Lowering eye pressure at least 30 percent through medicines slows the disease in some people. Glaucoma may worsen in others despite low pressures.
A comprehensive medical history is important in identifying other potential risk factors, such as low blood pressure, that contribute to low-tension glaucoma. If no risk factors are identified, the treatment options for low-tension glaucoma are the same as for open-angle glaucoma.
2.Angle-closure glaucoma. The fluid at the front of the eye cannot reach the angle and leave the eye. The angle gets blocked by part of the iris. People with this type of glaucoma have a sudden increase in eye pressure. Symptoms include severe pain and nausea, as well as redness of the eye and blurred vision. If you have these symptoms, you need to seek treatment immediately.
This is a medical emergency. If your doctor is unavailable, go to the nearest hospital or clinic. Without treatment to improve the flow of fluid, the eye can become blind in as few as one or two days. Usually, prompt laser surgery and medicines can clear the blockage and protect sight.
3.Congenital glaucoma. Children are born with a defect in the angle of the eye that slows the normal drainage of fluid. These children usually have obvious symptoms, such as cloudy eyes, sensitivity to light, and excessive tearing. Conventional surgery typically is the suggested treatment, because medicines may have unknown effects in infants and be difficult to administer. Surgery is safe and effective. If surgery is done promptly, these children usually have an excellent chance of having good vision.
4.Secondary glaucomas. These can develop as complications of other medical conditions. These types of glaucomas are sometimes associated with eye surgery or advanced cataracts, eye injuries, certain eye tumors, or uveitis (eye inflammation). Pigmentary glaucoma occurs when pigment from the iris flakes off and blocks the meshwork, slowing fluid drainage. A severe form, called neovascular glaucoma, is linked to diabetes. Corticosteroid drugs used to treat eye inflammations and other diseases can trigger glaucoma in some people. Treatment includes medicines, laser surgery, or conventional surgery.
Causes and Risk Factors
How does open-angle glaucoma damage the optic nerve?
In the front of the eye is a space called the anterior chamber. A clear fluid flows continuously in and out of the chamber and nourishes nearby tissues. The fluid leaves the chamber at the open angle where the cornea and iris meet. (See diagram below.) When the fluid reaches the angle, it flows through a spongy meshwork, like a drain, and leaves the eye.
Sometimes, when the fluid reaches the angle, it passes too slowly through the meshwork drain. As the fluid builds up, the pressure inside the eye rises to a level that may damage the optic nerve. When the optic nerve is damaged from increased pressure, open-angle glaucoma--and vision loss--may result. That's why controlling pressure inside the eye is important.
Does increased eye pressure mean that I have glaucoma?
Not necessarily. Increased eye pressure means you are at risk for glaucoma, but does not mean you have the disease. A person has glaucoma only if the optic nerve is damaged. If you have increased eye pressure but no damage to the optic nerve, you do not have glaucoma. However, you are at risk. Follow the advice of your eye care professional.
Can I develop glaucoma if I have increased eye pressure?
Not necessarily. Not every person with increased eye pressure will develop glaucoma. Some people can tolerate higher eye pressure better than others. Also, a certain level of eye pressure may be high for one person but normal for another.
Whether you develop glaucoma depends on the level of pressure your optic nerve can tolerate without being damaged. This level is different for each person. That's why a comprehensive dilated eye exam is very important. It can help your eye care professional determine what level of eye pressure is normal for you.
Can I develop glaucoma without an increase in my eye pressure?
Yes. Glaucoma can develop without increased eye pressure. This form of glaucoma is called low-tension or normal-tension glaucoma. It is not as common as open-angle glaucoma.
Who is at risk for glaucoma?
Anyone can develop glaucoma. Some people are at higher risk than others. They include:

■African Americans over age 40.
■Everyone over age 60, especially Mexican Americans.
■People with a family history of glaucoma.
Among African Americans, studies show that glaucoma is:
■Five times more likely to occur in African Americans than in Caucasians.
■About four times more likely to cause blindness in African Americans than in Caucasians.
■Fifteen times more likely to cause blindness in African Americans between the ages of 45-64 than in Caucasians of the same age group.
A comprehensive dilated eye exam can reveal more risk factors, such as high eye pressure, thinness of the cornea, and abnormal optic nerve anatomy. In some people with certain combinations of these high-risk factors, medicines in the form of eyedrops reduce the risk of developing glaucoma by about half.

Medicare covers an annual comprehensive dilated eye exam for some people at high risk for glaucoma.

What can I do to protect my vision?
Studies have shown that the early detection and treatment of glaucoma, before it causes major vision loss, is the best way to control the disease. So, if you fall into one of the high-risk groups for the disease, make sure to have your eyes examined through dilated pupils every two years by an eye care professional.
If you are being treated for glaucoma, be sure to take your glaucoma medicine every day. See your eye care professional regularly.
You also can help protect the vision of family members and friends who may be at high risk for glaucoma--African Americans over age 40; everyone over age 60, especially Mexican Americans; and people with a family history of the disease. Encourage them to have a comprehensive dilated eye exam at least once every two years. Remember: Lowering eye pressure in glaucoma's early stages slows progression of the disease and helps save vision.
Symptoms and Detection
What are the symptoms of glaucoma?
At first, there are no symptoms. Vision stays normal, and there is no pain.

However, as the disease progresses, a person with glaucoma may notice his or her side vision gradually failing. That is, objects in front may still be seen clearly, but objects to the side may be missed.

As glaucoma remains untreated, people may miss objects to the side and out of the corner of their eye. Without treatment, people with glaucoma will slowly lose their peripheral (side) vision. They seem to be looking through a tunnel. Over time, straight-ahead vision may decrease until no vision remains.

Glaucoma can develop in one or both eyes.

How is glaucoma detected?
Glaucoma is detected through a comprehensive eye exam that includes:

1.Visual acuity test. This eye chart test measures how well you see at various distances. A tonometer measures pressure inside the eye to detect glaucoma.
2.Visual field test. This test measures your side (peripheral) vision. It helps your eye care professional tell if you have lost side vision, a sign of glaucoma.
3.Dilated eye exam. Drops are placed in your eyes to widen, or dilate, the pupils. Your eye care professional uses a special magnifying lens to examine your retina and optic nerve for signs of damage and other eye problems. After the exam, your close-up vision may remain blurred for several hours.
4.Tonometry. An instrument (right) measures the pressure inside the eye. Numbing drops may be applied to your eye for this test.
5.Pachymetry. A numbing drop is applied to your eye. Your eye care professional uses an ultrasonic wave instrument to measure the thickness of your cornea.

Treatment
Can glaucoma be treated?
Yes. Immediate treatment for early stage, open-angle glaucoma can delay progression of the disease. That's why early diagnosis is very important.

Glaucoma treatments include medicines, laser trabeculoplasty, conventional surgery, or a combination of any of these. While these treatments may save remaining vision, they do not improve sight already lost from glaucoma.

1.Medicines. Medicines, in the form of eyedrops or pills, are the most common early treatment for glaucoma. Some medicines cause the eye to make less fluid. Others lower pressure by helping fluid drain from the eye.

Before you begin glaucoma treatment, tell your eye care professional about other medicines you may be taking. Sometimes the drops can interfere with the way other medicines work.

Glaucoma medicines may be taken several times a day. Most people have no problems. However, some medicines can cause headaches or other side effects. For example, drops may cause stinging, burning, and redness in the eyes. Many drugs are available to treat glaucoma. If you have problems with one medicine, tell your eye care professional. Treatment with a different dose or a new drug may be possible.

Because glaucoma often has no symptoms, people may be tempted to stop taking, or may forget to take, their medicine. You need to use the drops or pills as long as they help control your eye pressure. Regular use is very important. Make sure your eye care professional shows you how to put the drops into your eye. 2.Laser trabeculoplasty. Laser trabeculoplasty helps fluid drain out of the eye. Your doctor may suggest this step at any time. In many cases, you need to keep taking glaucoma drugs after this procedure.
Laser trabeculoplasty is performed in your doctor's office or eye clinic. Before the surgery, numbing drops will be applied to your eye. As you sit facing the laser machine, your doctor will hold a special lens to your eye. A high-intensity beam of light is aimed at the lens and reflected onto the meshwork inside your eye. You may see flashes of bright green or red light. The laser makes several evenly spaced burns that stretch the drainage holes in the meshwork. This allows the fluid to drain better.

Like any surgery, laser surgery can cause side effects, such as inflammation. Your doctor may give you some drops to take home for any soreness or inflammation inside the eye. You need to make several follow-up visits to have your eye pressure monitored.

If you have glaucoma in both eyes, only one eye will be treated at a time. Laser treatments for each eye will be scheduled several days to several weeks apart.

Studies show that laser surgery is very good at reducing the pressure in some patients. However, its effects can wear off over time. Your doctor may suggest further treatment.
3.Conventional surgery. Conventional surgery makes a new opening for the fluid to leave the eye. (See diagram.) Your doctor may suggest this treatment at any time. Conventional surgery often is done after medicines and laser surgery have failed to control pressure.

Conventional surgery is performed in an eye clinic or hospital. Before the surgery, you will be given medicine to help you relax. Your doctor will make small injections around the eye to numb it. A small piece of tissue is removed to create a new channel for the fluid to drain from the eye.

For several weeks after the surgery, you must put drops in the eye to fight infection and inflammation. These drops will be different from those you may have been using before surgery.

As with laser surgery, conventional surgery is performed on one eye at a time. Usually the operations are four to six weeks apart. Conventional surgery is about 60 to 80 percent effective at lowering eye pressure. If the new drainage opening narrows, a second operation may be needed. Conventional surgery works best if you have not had previous eye surgery, such as a cataract operation.

In some instances, your vision may not be as good as it was before conventional surgery. Conventional surgery can cause side effects, including cataract, problems with the cornea, and inflammation or infection inside the eye. The buildup of fluid in the back of the eye may cause some patients to see shadows in their vision. If you have any of these problems, tell your doctor so a treatment plan can be developed.
Conventional surgery makes a new opening for the fluid to leave the eye.
